This may sound a little drastic to some people, but when my daughter got to that stage [where she wouldn't lay still for diaper changes], I started changing her on the floor, and would lay her down between my legs, then put my ankles over her arms so she couldn't roll away. This is the only way I could change a poopy diaper without having her wiggle and get it all over the place.
